Fofana, 21, helped the Blues to their first three Champions League points of the season with a 3-0 win over Milan at Stamford Bridge.
The centre-back got off to a dream start, scoring the opening goal in the 24th minute, but his night turned sour soon after.
The France international suffered an apparent knee injury that brought him to tears just 14 minutes later.
The visibly emotional 21-year-old was unable to continue and was eventually replaced by Trevoh Chalobah.
Fans were clearly worried as they flooded Twitter fearing the worst for Fofana, who sustained a devastating broken leg at Leicester last year that kept him away for most of last season.
One fan tweeted: “I hope Fofana’s injury isn’t serious. His tears are not a good sign.”
Another wrote: “Very concerned for Fofana. In tears and it looks like the same knee he tore before. I really hope it’s minor.”
A third wrote: “Fofana’s injury really worries me. His knee twists as his cleats catch in the grass. Never good.”

This supporter explained, “This Fofana injury just sucks. A second long-term injury at the age of 21 could change his career forever.”
And another tweeted: “Dream come true for Fofana [his goal against Milan]. I really hope his injury isn’t serious and he can be back on the pitch soon.”
Chelsea sealed victory in the second half with goals from Pierre-Emerick Aubameyang after 56 minutes and Reece James just five minutes later.
