James “Whitey” Bulger, the organized crime boss who led the Irish mob in Boston in the 1970s and 1980s, was arrested in 2011.

He was then convicted of racketeering and conspiracy in August 2013 after a Boston jury found he was involved in 11 murders

In 2018, Bulger was transferred to Hazelton from a Florida prison, Coleman II.

The DOJ report found that while Bureau of Prison staff did not act with “malicious intent or for an improper purpose,” there were “serious job performance and management errors” that threatened its safety at Hazelton, reports FOX News.

The report noted that Bulger was approved to transfer to Hazelton despite his health issues.

According to the report, Florida prison health officials downgraded Bulger’s health assessment, leading to his transfer.

Part of the reason Bulger was transferred to Hazelton was because the prison “taken good care of the inmates,” the report revealed.

The report also said that over 100 Bureau of Prison officers were made aware of Bulger’s transfer.

Some spoke to Hazelton inmates about the upcoming move, and the media had also reported on the move.

However, some Bureau of Prison officials were unaware of Bulger’s reputation and did not consider his identity when planning the transfer.

“Our investigation found serious underperformance by BOP staff and management at multiple levels, bureaucratic incompetence, and flawed, confusing and inadequate BOP policies and procedures,” said Justice Department Inspector General Michael Horowitz.

“We found it particularly worrying that Coleman’s staff placed Bulger, an 89-year-old inmate who used a wheelchair and suffered from severe heart problems, in a solitary confinement cell in Coleman’s Special Housing Unit (SHU) for eight months while Coleman’s staff were with faced bureaucratic difficulties in getting him transferred to a new facility.

“The fact that these serious deficiencies were occurring in the context of a high-profile inmate like Bulger was particularly concerning given that the BOP would presumably take special care in handling such an inmate case,” Horowitz continued.
