The police car was chasing an Audi A3 that officers believed was stolen when they fatally crashed into Heather.
The Independent Office for Police Conduct (IOPC) said an unnamed police officer was being investigated for a possible crime of causing death by dangerous driving.
They will also be investigated for possible negligent driving and gross misconduct related to their driving.
Heather was treated by paramedics but was pronounced dead at the scene about 30 minutes after the accident.
Two police officers were also treated for injuries sustained in the collision.
IOPC Regional Director Catherine Bates said: “Ms Smedley died in tragic circumstances just days before Christmas.
“Our thoughts are with her family and everyone affected by her death.
“Our investigation is independent of the police and will examine the circumstances of the collision, including the actions and decisions of the police driver.
“We have been in contact with Ms Smedley’s family to explain our role and will keep them informed as our investigation progresses.”
Heather’s family paid tribute to her after her death.
They said: “To Benjamin’s most wonderful mother, Olivia and Jamie, daughter of Hilda and Fred, sister, friend and wife of Damian.
“The world was a much better place with you.
“Our mother was the kindest, gentlest and most beautiful soul inside and out.
“We couldn’t have asked for a better role model as she grew up giving everything she had for everyone she loved and always supporting those around her.
“You were too good for this world mom, it will forever be a hole in our hearts.”
