Cook, 31, returned to the club for the first time after spending a decade after joining Steve Cooper’s side in January.

However, his evening began under the most difficult of circumstances, when his father had to be brought back to life by paramedics.

Following his side’s 1-0 defeat which sealed promotion for Bournemouth, Cook wrote on Twitter: “Firstly big congratulations to @afcbournemouth on promotion but tonight I have to thank the paramedics at the game.

“Unfortunately my father went into cardiac arrest just before the game and they managed to bring him back to life.

Bournemouth wrote on social media: “Thinking of you Cookie.

“fingers crossed for a speedy recovery ❤️”

Despite the incredibly difficult news, Cook started for Forest at Vitality Stadium.

He was also in the wars and needed treatment and stitches after a nasty first-half clash.

Cherries’ old boy Sam Surridge had a chance to put Forest ahead but shot off the bar.

It was Kieffer Moore who scored the winning goal, getting up off the bench in the 83rd minute to convert a clever Phillip Billing free-kick.

Cook made 387 appearances for Bournemouth before leaving earlier this year and scored 21 goals for the Cherries.

These outings included 168 in the Premier League.

Earlier this season, he played three league games for Scott Parker’s side before joining Forest’s play-off team.
