One driver, a 12-year-old girl, reportedly remains hospitalized in Naples after the incident.
Dramatic phone video footage of the drama shows the chair swing in operation before its base begins to wobble uncontrollably.
As the ride begins to rise, spectators begin to flee and the swing chair tips on its side, scattering its passengers.
Local police are now said to be investigating the accident at the Madonna delle Grazie (Our Lady of Graces) festival.

They confiscated the fairground ride, and investigators currently believe structural failure may be to blame.
They are investigating a person allegedly responsible for the malfunction of the fairgrounds – a woman from nearby Poggiomarino – on charges of negligent injury.
They confiscated papers from her describing the fairground’s insurance policy and the last technical test of the carousel.
The mayor of Palma Campania, Nello Donnarumma, reported that the incident happened just a few hours before the planned dismantling of the fairgrounds.
The incident comes after three children were taken to hospital after a fairground ride was snapped in mid-air at Ataturk Park in Antalya, south-west Turkey.
Footage showed the children’s faces turning from joy to panic as they were locked in their seats as the ride stalled just moments after takeoff.
One of the injured youths said the ride “fly through the air” after it broke.
He told local media: “While we were swinging, a cable broke or something.
“The vehicle flew through the air and landed upside down.
“I hit my head and I also have bruises on my foot.”
Meanwhile, another drama captured the moment fairgoers were left hanging upside down 130 feet off the ground when a ride was cancelled.
Fun-seekers were stuck for up to 40 minutes when a new ride at a Russian amusement park broke during testing.
The ride, dubbed ‘Booster’, collapsed at Kyrlay amusement park in the city of Kazan, southwest Russia, last summer.
