The result saw them relegated from League A of the Nations League for the first time ever.
After a stunning first half from both sides, Italy broke the deadlock in the 68th minute thanks to a fantastic goal from Giacomo Raspadori.
The striker brought the ball to the edge of the box from a Leonardo Bonucci cross before cutting inside and unleashing a superb shot into the bottom right corner.
Southgate reacted quickly with a double substitution.
But it was too little too late as the Three Lions suffered their third loss to the competition.
Even if they beat Germany on Monday, they will still be bottom of Group B.
The fans at the San Siro were furious after the final whistle and the majority booed the English team off.
SunSport questioned Southgate about England fans booing the team. He simply replied, “It’s understandable.”

Many also took to Twitter to respond by writing: “The team is completely lost at the moment. Southgate’s stubbornness is ruining this team and draining all the players’ trust.”
Another added: “This is embarrassing.”
And a third said, “Wow, just wow, get him out now!”
England have not scored from open play in their last five games.
They are now going five straight games without a win for the first time in EIGHT years.
Speaking to Channel 4 after the game, midfielder Declan Rice said: “Obviously it’s disappointing.
“Every tournament we go to, we want to win.
“We fell below our standards in the Nations League, but I didn’t think everything was bad tonight.”
